Lead Diabetes Specialist Nurse

This is an opportunity to join a fast-growing health care company using digital tools to address some of the largest challenges facing healthcare and improve patient outcomes. You will be using your clinical knowledge and experience to provide guidance for patients with diet related health conditions such at type 2 diabetes, prediabetes, and obesity. The role involves a variety of; remote patients’ consultations, contributing to company growth strategy and development and supporting business development opportunities.

What you'll be doing:

• Support the UK Medical Lead in the design of company’s medical pathways for weight management and type 2 diabetes services.

• Understand the UK diabetes and obesity market supporting the internal Partnership and Clinical Expert team in increasing the accessibility of the service offered.

• Input into internal and external research strategy.

• Delivery of gold-standard remote (email or phone) care to patients living with obesity and/or type 2 diabetes. Providing both one to one patient consultations and delivering remote GLP-1 patients education groups.

• Oversee patient handovers, communicating with local GP practices, obesity and diabetes teams and practice nurses.

• Ensure parallel working with the Diabetes Specialist Dietitians to provide full patient support.

• Work autonomously with management of clinical caseload and projects.

What experience you will need to apply:

• A minimum of 5 years’ experience as a registered nurse supporting patients with obesity and type 2 diabetes. A confident prescriber.

• Patient focused with a desire to improve health outcomes via digital support and behaviour modifications. Ability to adapt to a changeable and busy environment – prioritising and multitasking.

• Experience withing web-browser technology to record and review clinical patient notes.

• Strong communication skills (including virtual communication) and the ability to represent the company with professionalism. Strong team player.
